Since May 2026, patients with a long-term condition (ALD) have been able to designate a reference nurse with the Health Insurance fund. The goal is to improve care coordination and follow-up for patients with chronic conditions. What is the role of the reference nurse and how do you designate one? Service Public informs.

This new scheme implemented in May 2026 concerns patients with so-called exempting long-term conditions (ALD). These are ALDs requiring prolonged treatment and particularly expensive therapy, covered up to the maximum reimbursement by the Health Insurance fund (exemption from the co-payment).
The role of the reference nurse
A decree of May 22, 2026 defines the missions of the reference nurse. They ensure a role of care coordination for the patient. They act as a liaison between the various healthcare professionals involved in the patient's care pathway, notably the treating physician and health and medico-social establishments. They are required to update the patient's shared medical record (DMP) and record all data necessary for their follow-up.
As of July 1, 2027, the reference nurse will be called upon to intervene within the framework of enhanced follow-up for certain patient profiles, such as the elderly or those with disabilities, who cannot access medical consultations without the aid of sanitary transport, in particular.
Who can declare a reference nurse?
The designation of the reference nurse is based on the patient's free choice and the practitioner's agreement.
The scheme is open to:
-
patients aged 16 or over suffering from an exempting ALD;
-
dependents, under the same conditions;
-
minors, with the consent of at least one holder of parental authority.
Only one nurse can be designated as the reference. However, nurses practicing within the same office may substitute for each other in the absence of the reference nurse if the situation requires it.
How to declare a reference nurse?
The declaration is made during an in-person visit. You complete the paper form for Declaration of Choice of a Reference Nurse with the chosen practitioner. This document is co-signed by you (or your representative) and the nurse. The nurse then sends the paper form by post to your Primary Health Insurance Fund (CPAM).
Remember to prepare 3 copies of this form: one for you, one for the nurse, and one for the CPAM.
It is possible to change your reference nurse at any time. The CPAM will notify you and the nurse of the change.
